When an individual has passed, the next-of-kin must sign a release form to authorize Chevra Kadisha Mortuary to transport the deceased. We must also be provided with the deceased's vital statistics for the death certificate.
All forms can be completed and e-signed online via Adobe Sign, or downloaded as a Word/PDF document.
Please have the following information ready when contacting us:

If the deceased is in the custody of a county medical examiner or coroner, please use the appropriate form below.
